I am going to attempt to organize and track D’s progress here…
Visit # 1- Occupational Therapy
A play session. Notes: Today’s focus was pairing. D was able to tolerate therapist in the room and engage in play with the help of Mom. D displays decreased coordination of his movement and visual systems. This is why he places his head on the floor and looks at things peripherally. He also displays decreased upper body stability. Encourage D in crawling, bear walking and wheel barrow walking. Have D participate in gross motor play and strengthen his core musculature to begin to coordinate vision with movement.
